Connecting Communities Collective

Healthy Options is a founding member of the Connecting Communities Collective, along with AliEnergy, Hope Kitchen and Oban Youth Cafe. Atlantis Leisure Centre is also a member.

The CCC grew from the understanding that the whole is greater than the sum of its parts; by working together, essential sector organisations can better use resources, share learning, and reach more people in our communities. We aim to strengthen capacity for partnerships, and are working to encourage and support more connections.

Oban Living Well

Oban Living Well is a collaborative approach to health and wellbeing between Healthy Options, the NHS and local third sector organisations. We are a key component of the Oban Living Well Support Service Model - click on the image to view in more detail. We prevent or slow down the progression from blue to yellow, yellow to orange and from orange to red.

Other Partnership Working

Lorn Medical Centre:
On a weekly basis we take part in multidisciplinary meetings with the local Medical Centre where referrals onto our Reablement Pathway are discussed and agreed.

Physiotherapy Department - Lorn & Islands District General Hospital:
S
ince 2016 we have been working in partnership with the Physiotherapy Department at the local hospital, predominantly for frailer members of the community who were at risk of requiring care packages or at risk of requiring admission to hospital (see orange band on the Oban Living Well model above).  We are now developing a new level of partnership working to cover clients requiring re-hab or pre-hab and those with neurological conditions.

We sit on the Living Well Strategy subgroups within HSCP (NHS’ Health and Social Care Partnerships) and third sector organisations such as LIVE Argyll, Versus Arthritis, Third Sector Interface, North Argyll Carers Centre and the MS Centre in mid Argyll.

In addition to this we have ever-growing referral and prescription relationships with many public, private and third sector organisations throughout the Lorn area.
